Carmichael, 2009, Type information as given in the original paper. There is no mention of how many specimens were looked at. Although since he described both winged and wingless stages, it can be assumed that there is more than one specimen. However, it is not clear if he retained any of the type specimens, since it is noted that the descriptions, including the microscopic view, were based on living specimens. There is no mention of a depository. The host is Caryae alba (Shell bark hickory), the collection date is June 7th -20th, by H. Shimer, from Mount Carrol, Illinois.
